Clinical Study of Polymyositis-like syndrome associated with Liver Cirrhosis

Abstract

|
Polymyositis is an inflammatory muscular disease of unknown etiology Recently. we experinced eighteen patients with polymyositis-like syndrome associated with liver cirrhosis. General muscular ache, mucsle weakness, hematuria and the marked
elevation of
CK and LDH level were characteristic clinical findings. They had more poor prognosis than liver cirrhosis patients without polymyositis-like syndrome. Six of eighteen patients had combined hepatocelluar carcinoma. Eight of eighteen patients died
soon
after admission.
Six of eight dead patients was related to HBV-induced liver cirrhosis, In survivals. The level of CK and LDH decreased to normal value and symptoms were relived. Poor prognostic factor might be severe cirrhotic state, hepatitis B induced
cirrhosis,
female and old age.
The causes of polymyositis-like syndrome in cirrhosis were not known but we suspected that they might be related to malignancy. Combined autoimmune state(typelll, l or V) or viral infection.
We present the cases with review of literature and this may be the first report in Korea.
